In submitting this application, I understand and agree to the following:

I attest that all of the information I have provided on this application is true. I agree to serve as a productive member of the foster program. I will follow Hannah’s policies and procedures explained to me during the foster parent orientation and as described in the Foster Parent Handbook.

I agree that if my foster pet requires medical attention, I will bring it to a Hannah location for care. I understand that if I seek veterinary care for a foster pet outside of Hannah without pre-approval from Hannah, I will not be reimbursed for any expenses I incur.

I give my consent to Hannah to provide my name, voice and/or photograph, or that of any pet I care for, to the media in connection with advertising, programming or operational activities for Hannah. I agree to hold harmless Hannah’s, and any of its agents, employees, directors, and insurance carriers from all actions, claims of every nature, damages or judgments in matters relating to my service as a Hannah Foster Parent. This includes, but is not limited to, personal injury.

Nearly two weeks ago I took my dog to another clinic and she was diagnosed with Pyometra and I was told she would die that day without surgery. I couldn't afford the $3,800 they wanted. . . A week later and my fur baby was still alive and fighting. Finally I ran across Hannah. . . From the moment I walked in with my insanely thin and ill dog, I was treated with respect and compassion. They didn't hesitate to take care of my fur baby, blood work, X-ray, IV with fluids before I even gave them payment information. My dog is alive, back home, after days of surgery and hospital care, for a fraction of the cost the other vet wanted, and payments spread over the next 12-months. I also, now, will never have to experience this again because for my monthly fee and membership my dog will have great veterinarian care for the rest of her life, no matter what financial obstacles are thrown at me. . .
